Abstract Scand J Caring Sci; 2012 Adaptation to Swedish and further development of the 'Consequences of Screening - Breast Cancer' questionnaire: a multimethod study Rationale: Experiencing a false-positive screening mammography can cause considerable psychosocial distress. The Consequences of Screening - Breast Cancer questionnaire (COS-BC parts 1 and 2), recently developed in Denmark, is the only condition-specific questionnaire for measuring short- and long-term psychosocial consequences of false-positive mammographic screening. Additional studies are needed to further test the COS-BC before use across cultures. Furthermore, studies have suggested that the consequences of false-positive screening results are partly common across cancer screening settings, although this hypothesis remains largely untested. Objectives: This study (i) assesses content validity of a Swedish version of the COS-BC, (ii) tests whether items expressing long-term consequences of false-positive lung cancer screening results are relevant in a breast cancer screening context and (iii) explores the usefulness of taking results from Rasch analyses of the source version as an aid in questionnaire translation and adaptation. Methods: Following dual-panel translation, content validity was assessed through qualitative interviews with representatives of the target population and the content validity index (CVI). Item locations and Rasch model fit of the source questionnaires were considered in the translation and assessment process. Results: The COS-BC items were generally found relevant and provided coverage of the target construct. Content validity was supported also for nine of 10 lung cancer screening items. Scale CVI values were ≥0.81. Previous Rasch data were useful in facilitating translation and assessing item content validity. The resulting Swedish version of the COS-BC parts 1 and 2 consists of 34 and 23 items, respectively. Conclusion: This study illustrates the value of methodological triangulation and use of data from previous Rasch analyses in questionnaire translation and adaptation. English abstract:

Rationale: Cross-cultural comparative studies using reliable and valid instruments can increase awareness of the differences and similarities between health worker's ability to respond to patients' individual needs within different health systems. This will enable a better understanding of cultural perspectives in individualized nursing care. Aim: To describe the translation and adaptation process of the Individualized Care Scale (ICS) and examine its reliability and validity in a cross-cultural study. Design: A cross-sectional comparative study. Settings: Twenty-seven orthopaedic and trauma in-patient units at 14 hospitals in 5 countries. Participants: A total of 1126 patients were included in the study: Finland (n = 425), Greece (n = 315), Sweden (n = 218), UK (n = 135) and USA (n = 33). Methods: A systematic forward- and back-translation procedure using bilingual techniques, a committee approach, pretest techniques and pilot testing were used with a convenience sample to produce a valid ICS for each participating group. Psychometric evaluation of the adapted ICS was based on means, SD, missing data analysis, Cronbach's alpha coefficients and average inter-item correlations. Construct validity was examined using sub-scale correlations to total scales and principal components analysis. Results: The use of the range of options and the sub-scale mean scores ranging from 2.72 to 4.30 demonstrated the sensitivity of the scale. Cronbach's alpha coefficients (0.77-0.97) and average inter-item correlations (0.37-0.77) were acceptable. The sub-scale correlations to total scales were high (0.83-0.97). The underlying theoretical construct of the ICS was demonstrated by the explained variances ranging from 58% to 79%. Conclusions: The ICS shows promise as a tool for evaluating individualized care in European cultures. The international expansion of an existing instrument developed for one country facilitates comparative studies across countries. There is a need to further test the construct validity and appropriateness of the ICS in different settings in European and nonwestern cultures.

Irritable Bowel Syndrom (IBS) är en vanlig global funktionell mag- och tarmsjukdom och uppskattningsvis har 12 procent av Sveriges befolkning IBS. Patienterna behandlas av läkare i primärvården eller av specialistläkare. IBS kännetecknas av besvär från mag-tarmkanalen utan någon bakomliggande påvisbar patofysiologisk eller biokemisk orsak och diagnosen baseras på Rom III kriterierna. Patienter med IBS kan beroende på besvär ha diarré- respektive förstoppningsdominerad IBS eller en intermittent avföringsfrekvens som växlar mellan diarré och förstoppning. Orsakerna till patienternas besvär är komplexa och det är flera faktorer som kan ha betydelse för sjukdomens uppkomst. Störningar i tarmmotoriken, ökad känslighet i tarmen s.k. inre hypersensivitet, störningar i kommunikationen mellan tarmen och hjärnan s.k. ”brain-gut axis”, samt störningar i hormoner och peptider har visat sig ha en viss betydelse. Det är fler kvinnor än män som drabbas av IBS och symtomen varierar beroende på kön. Förstoppning till exempel, är vanligare bland kvinnor, medan diarré är vanligare bland män. Patienter med IBS lider också av många andra besvär, såsom gaser, trötthet, illamående och huvudvärk. Patienter som har IBS har en hög sjukfrånvaro och de utsätts för stort lidande, vilket i förlängningen även påverkar deras sociala situation och dagliga liv. Samhällets kostnad för patienter med IBS är stor eftersom de konsumerar mycket sjukvård i sin jakt efter hjälp för sina besvär. Vid läkarbesöket är det patientens uppgifter som får ligga till grund för fortsatt utredning, men någon enhetlig utrednings- och behandlingsstrategi finns inte. Omhändertagandet av patienter med IBS är i huvudsak medicinskt inriktad och syftar till att lindra patientens besvär och öka välbefinnandet, eftersom någon botande behandling inte finns. Omvårdnadsåtgärder saknas vilket kan anses vara en brist, speciellt för denna patientgrupp som har en så komplex sjukdomsbild. Det fanns ett behov av ett kort och enkelt bedömningsinstrument för att få en uppfattning om de besvär som patienter med IBS har och för att följa behandlingseffekten. Därför utvecklades Visual Analogue Scale for Irritable Bowel Syndrome (VAS-IBS) för att fånga patienters subjektiva upplevelse av sina mag- och tarmbesvär. Formuläret består av nio frågor varav sju skall skattas av patienten med hjälp av en visual analog skala (VAS) och två besvaras med JA/NEJ. De psykometriska testerna bekräftade att VAS-IBS har hög tillförlitlighet avseende validitet, reliabilitet och intern samstämmighet. Formuläret är enkelt att använda för patienter såväl som för vårdpersonal.

English abstract:	Information and education are important components in the care of patients before colorectal surgery. In order to optimize a quick recovery after surgery sufficient and evidence based information is essential. The aim was to describe and review scientific articles regarding the importance of the preoperative patient information given before colorectal surgery from a patient perspective. The method was a literature study, with a systematic approach, in order to find evidence concerning patient information before colorectal surgery. The results showed that patients describe experiences of anxiety, insecurity, and the fear of the unknown. They needed healthcare givers which provide a feeling of security and continuity. If the patients receive multidimensional, written and verbal information, they are less anxious and have higher self-control postoperatively. They also experience less pain and can be mobilized earlier. If patients who should receive a stoma met a stoma therapist preoperative expressed less anxiety, fewer stoma complications and better mental well-being. This means that the patients can manage their colostomy earlier. Accurate preoperative information enhances understanding of the postoperative recovery phase and makes patients more involved. This leads to earlier recovery and discharge. The conclusion is that patients undergoing colorectal surgery are in great need of preoperative information. When this is given due to a standard care program, with regard to the individual needs, the patients will be better prepared for their surgery and postoperative care. Patients will participate in their care actively and take responsibility early, reducing complications and accelerate the post-operative recovery. Through good cooperation in the team around the patient, the individual needs of the patient are optimized. If the patient is assigned a patient charge nurse, the individual needs is identified earlier and if necessary followed up postoperatively after discharge.
Swedish abstract:	Information och utbildning är viktiga delar av vården inför colorektal kirurgi. Att ge evidensbaserad information som förberedande och tillräcklig för att patienterna skall kunna hantera den postoperativa tiden till snabb återhämtning är en förutsättning. Syftet var att beskriva och sammanställa vetenskapliga artiklar avseende betydelsen av den preoperativa patientinformationen som ges inför colorektal kirurgi ur ett patientperspektiv. Metoden var en litteraturstudie för att systematiskt finna evidens för patientinformation inför colorektal kirurgi med systematisk ansats. Resultatet visade att patienter beskriver den preoperatia upplevelsen som oro, osäkerhet, och stress inför det okända. De har behov av vårdgivare som genom mötet inger säkerhet och kontinuitet. Då patienterna får flerdimensionell, skriftlig och muntlig information är de mindre oroliga och har högre självkontroll postoperativt. De upplever också mindre smärta och kan mobiliseras tidigare. Om patienterna som skall erhålla stomi träffat en stomiterapeut preoperativ leder det till minskad oro, färre stomikomplikationer samt ett bättre psykiskt välbefinnande. Detta göra att patienterna kan hantera sin stomi tidigare. Noggrann preoperativ information ökar förståelsen av den postoperativa återhämtningsfasen och gör patienterna mer delaktiga, vilket leder till tidigare återhämtning och hemgång. Konklusionen är att patienter som ska genomgå colorektal kirurgi är i stort behov av preoperativa information. Då denna ges utifrån ett standardiserat vårdprogram med beaktande av individuellt behov blir patienterna bättre förberedd på sin operation och den postoperativa vården. Patienterna delta aktivt i sin vård och tar ansvar tidigt, vilket minskar komplikationerna och påskynda den postoperativa återhämtningen. Genom ett bra samarbete i teamet runt patienten kan de individuella behoven hos patienten optimeras. Om patienten tilldelas en patientsansvarig sjuksköterska kan det individuella behovet identifieras tidigare och vid behov följas upp postoperativt efter utskrivning.
