English abstract: The objective was to perform a long-term follow-up study of patients that had received high cost dental care within the Swedish National Dental Insurance System in 1977-1978 with special focus on remaining teeth, periodontal disease progression, change in the prevalence of root-filled teeth and teeth with apical periodontitis as well as the survival of fixed prosthetic reconstructions. All 262 patients who had had their treatment plans sent for approval for high-cost dental care in 4 local health insurance districts and who were sampled for base-line studies in 1977-1978, were offered a free clinical examination including radiographs in 1998. 177 patients (68% of the original sample) could be reached for telephone interview and 104 of them (40% of the original sample) were examined clinically and radiographically. Comparisons were made with records and radiographs from 1977-1978. The analyses were performed with the individual patient as the studied unit. The low progression of severe periodontal disease during the 20-23 year follow-up period and the decrease in number of teeth with apical periodontitis among a majority of the patients examined, indicated that the dental care received resulted in a limitation of dental disease on the individual level. Furthermore 63% of the patients had the fixed prosthetic reconstructions, received after approval 1977-1978, in full extention after 20-23 years. However, more tooth losses were observed among the patients in this study than in similar studies in Swedish general populations over the same decades. Furthermore multiple tooth extractions were significantly more frequent in patients with severe periodontitis at baseline and in patients with less apical periodontitis at follow-up in this study. Thus it seems that tooth extraction not seldom was a treatment choice for teeth with severe periodontitis and apical periodontitis among the patients examined clinically in this study. -

English abstract: OBJECTIVES: To develop a digital densitometric tool for jaw bone to analyse intraoral radiographs. To assess precision and accuracy for this tool and determine the minimal detection threshold for density changes. METHODS: Bone samples deriving from the premolar region of 47 human mandibles were selected for analysis. The samples were obtained from adult cadavers in the department of anatomy (Faculty of Medicine, KULeuven) with ethical approval. Digital radiography was performed on all bone samples. Direct volumetric measurements served as gold standard density values and allowed determination of accuracy. Dual-energy X-ray absorptiometry (DXA) scans were performed on all specimens. For all radiographs, density in mm Al eq was calculated using custom-made software, Osteop. Precision and intraobserver and interobserver reliability of this method were assessed. The bone specimens were progressively decalcified. At standard time intervals the percentage of decalcification was calculated. At each decalcification step, radiographs were taken and analysed. RESULTS: CV was always lower than 3%, which points to a good precision of the method. Correlation between the density measurements in mm Al eq and the DXA results was 0.9, for the density measurements in mm Al eq and the direct density measurements r was 0.5. The custom-made software was able to detect a change in bone mineralization of 6.6%. CONCLUSIONS: The present method for bone densitometric analysis offers potentials for clinical evaluation of bone density and minute bone density changes in the jaw bone. -

English abstract: Clinical questionnaires and dental radiographic findings have both been suggested as methods of identifying women at risk of having osteoporosis and who might benefit from bone densitometry. The aim of this study was to measure the diagnostic accuracy of a combination of mandibular cortical width (MCW) measured from dental panoramic radiographs (DPRs) and the osteoporosis index of risk (OSIRIS) in the diagnosis of osteoporosis. 653 women (age range 45-70 years, mean age 54.95 years) in four European centres underwent standardised dual X-ray energy absorptiometry (DXA) to provide reference data on osteoporosis status. Each subject was interviewed to derive OSIRIS scores and underwent DPR examination. MCW was measured directly by five observers. Receiver Operating Characteristic (ROC) curve analysis was used to calculate sensitivities and specificities of the clinical and radiographic tests for the diagnosis of osteoporosis. 512 (78.4%) of the study population were classified as having normal BMD and 141 (21.6%) as having osteoporosis. Using ROC analysis, OSIRIS gave a ROC curve area (A(z)) of 0.838, with a sensitivity of 70.9% and a specificity of 79.5% at a diagnostic threshold of <or=+1. MCW on DPRs gave Az values for the five observers ranging from 0.71 to 0.78, providing sensitivities between 41.0% and 59.6% and specificities of between 81.8% and 90.3% at a diagnostic threshold of a 3 mm MCW and sensitivities between 94.2% and 99.3% and specificities of between 9.8% and 23.7% at a diagnostic threshold of a 4.5 mm MCW. Inter-observer repeatability was less than 2.15 mm for 95% of subjects. Combining clinical and radiographic tests had the effect of improving specificity at the expense of a fall in sensitivity. Diagnostic thresholds for MCW and OSIRIS can be chosen to provide the sensitivity and specificity combination that best suits locally determined needs. However, the addition of OSIRIS as a stepwise 'follow-up' test to radiographic assessment of MCW should only be performed if the aim is to have a test for which the highest achievable specificity is desired. -

English abstract: Objectives: To study the diagnostic accuracy of the clinical examination of the Research Diagnostic Criteria (RDC) and of the dynamic and static pain tests for the recognition of temporomandibular disorder (TMD) pain and to improve the RDC accuracy by 1) changing the myofascial pain cutoff of 3 painful muscle palpation sites, or 2)omitting unreliable palpation sites. Methods: In 4 European dental faculties, a blind examination was performed in 125 chronic TMD pain patients, 88 chronic dental pain patients, and 121 pain-free subjects. Allocation was based upon the results of an oral history and a dental examination. Results: Sensitivity and specificity of the RDC were .88 and 45-.71, respectively. Increasing the myofascial pain cutoff better met the recommended levels for specificity and sensitivity of .70 and .90, respectively. When unreliable muscle palpation sites (i.e., the intraoral and submandibular sites) were omitted, the accuracy of the RDC/TMD examination did not change. For the dynamic and static pain tests, sensitivity (.65) and specificity (.84-.91) did not differ significantly from the recommended levels. Conclusion: A suspected TMD origin of orofacial pain is best confirmed by pain on the dynamic or static tests, while it is better denied by a negative outcome of the RDC examination. The intraoral and submandibular palpation sites of the RDC examination do not contribute to its diagnostic accuracy and can better be omitted, while the cutoff for a myofascial pain diagnosis should be increased. -

English abstract: The aim was to assess the influence of culture on pain expression, pressure pain thresholds (PPTs), and pain tolerance levels (PTLs) in temporomandibular disorders (TMD) pain cases and pain-free matched controls in three cultures. Methods: This was a case-controlled study on 150 (50 Saudi Arabians, 50 Italians and 50 Swedes) cases of chronic TMD pain compared with 148 age- and gender-matched pain-free controls (50 Saudi Arabians, 50 Italians, and 48 Swedes). The cases and controls completed pain questionnaires and underwent clinical examinations per the Research Diagnostic Criteria for TMD (RDC/TMD) for classification status. PPT and PTL were measured on all participants with a pressure algometer (Somedic®) at a pressure increase rate of 30 kPa/s using a 1.0-cm diameter probe. Three body sites on the right side were investigated: the temporalis, masseter, and thenar muscles. The average of three measurements made at 1-min intervals was calculated for PPT and PTL. A one-way ANOVA compared mean values. Results: Mean characteristic pain severity among TMD cases was 54±(25) for the Saudi Arabian; 56±(20) for the Swedish; and 62.5±(21) for the Italian cases. Between-culture differences were non-significant. PPTs at the temporalis and masseter muscles in the TMD cases were highest in the Saudi Arabians compared to the Swedes and Italians (P<0.001). No between-culture differences among the TMD cases were found in the thenar muscle. Among controls, higher PPTs in the masseter muscle were found in Swedes and Saudi Arabians compared with Italians (P<0.001), while Swedes reported the highest PPT for the thenar compared with Saudi Arabians and Italians (P<0.001). No between-culture differences were observed at the temporalis muscle. PTLs at the masseter muscle in the TMD cases were similar for the Saudi Arabians and Swedes but significantly lower for the Italians (P<0.001). Thenar PTLs were significantly higher in the Swedes compared to the Italians (P<0.001). No significant between-culture differences among the TMD cases were found in the temporalis muscle. Among controls, similar PTLs were observed in the Swedes and Saudi Arabians for the temporalis and masseter muscles, while the Italians had significantly lower PTLs than these groups at the masseter muscles (P<0.01) and lower than the Swedes at the temporalis (P<0.001). A significantly higher thenar PTL was found among Swedes compared to Saudi Arabians and Italians (P<0.001). Cross-modality PPT based on clinical pain for TMD cases revealed significant differences between Saudi Arabians and both Swedes and Italians in the temporalis and masseter muscles (P<0.001). No significant differences were found in the thenar muscle. Conclusions: Preliminary data suggest that Saudis and Swedes are more similar in PPT and PTL than Italians are to either group, indicating that cultural factors may influence pain perception. -

English abstract: Repeated intramuscular injections of acidic saline in rats are reported to induce a chronic type of muscle sensitization without causing significant tissue damage in this model two repeated injections of acidic saline (pH 4) into the gastrocnemius muscle produce short-lasting pain, but allodynia that lasts for 4 week with pain spread to the contralateral side. This may indicate that central sensitization occurs and this model is thus believed to more accurately mimic chronic myalgia. Our aim was to find out if repeated injection of buffered acidic saline into the masseter muscle causes pain and hyperalgesia and if so this would be a valid experimental model for orofacial myalgia / myofascial TMD in humans. Twenty healthy and pain-free subjects (10 male + 10 female: 25.1 ± 0.9 years) were included. Pain levels were assessed on a 0-10 numerical rating scale (NRS) and pressure pain thresholds (PPTs) were recorded using an electronic pressure algometer. The subjects received a unilateral intramuscular injection (0.5 ml) of acidic saline (pH 3) or placebo (isotonic saline, pH 6) into the masseter muscle (randomized and double-blind). Two days thereafter the injection was repeated with the same substance. Pain was assessed 5 min before the injection and at 5, 15, 30, 45 min, 1, 3, 24 hours after both injections and then at 4 and 7 days after the second injection and PPTs at 5, 15, 30, 45 min, 1 hrs after both injections and at 1, 4 and 7 days after the second injection. The experiment was repeated with the other substance after at least one week. There was no difference between substances in NRS pain scores after the injections. One way ANOVA indicated a time effect but no substance effect on PPTs. These preliminary results do not support acidic saline injections as a valid pain model for experimental masseter myalgia in healthy subjects. -

English abstract: Streptococcus mutans, a member of the dental plaque community, has been shown to be involved in the carious process. Cells of S. mutans induce an acid tolerance response (ATR) when exposed to sublethal pH values that enhances their survival at a lower pH. Mature biofilm cells are more resistant to acid stress than planktonic cells. We were interested in studying the acid tolerance and ATR-inducing ability of newly adhered biofilm cells of S. mutans. All experiments were carried out using flow-cell systems, with acid tolerance tested by exposing 3-h biofilm cells to pH 3.0 for 2 h and counting the number of survivors by plating on blood agar. Acid adaptability experiments were conducted by exposing biofilm cells to pH 5.5 for 3 h and then lowering the pH to 3.5 for 30 min. The viability of the cells was assessed by staining the cells with LIVE/DEAD BacLight viability stain. Three-hour biofilm cells of three different strains of S. mutans were between 820- and 70,000-fold more acid tolerant than corresponding planktonic cells. These strains also induced an ATR that enhanced the viability at pH 3.5. The presence of fluoride (0.5 M) inhibited the induction of an ATR, with 77% fewer viable cells at pH 3.5 as a consequence. Our data suggest that adhesion to a surface is an important step in the development of acid tolerance in biofilm cells and that different strains of S. mutans possess different degrees of acid tolerance and ability to induce an ATR. -

English abstract: In nature, bacteria are organized in surface-associated biofilm communities. Dental plaque is one of the most studied biofilms and harbours many different species including the star of this thesis Streptococcus mutans. S. mutans has been implicated in the aetiology of caries and has been studied extensively but most research has been carried out on liquid cultures (planktonic cells). Biofilm cells have been shown to differ in many ways from planktonic cells when it comes to protein and gene expression, growth rate, and stress tolerance. The overall aim of this thesis was to study S. mutans grown in biofilms of different ages (2 hours – 3 days) and to study the acid tolerance response (ATR) in biofilm cells. Biofilms were grown in chemostats, on glass slides and in flow-cell systems. To evaluate protein expression in cells, 14C-labelled proteins were separated by 2-dimensional gel eletrophoresis (2-DE) followed by autoradiography and computer-assisted image analysis. Acid killing experiments were conducted by exposing the cells to pH 3.0 for two hours and then counting the number of survivors by plating on blood agar. ATR-experiments were carried out in a similar way to the acid killing experiments except that the cells were first exposed to pH 5.5 for 2-3 hours and then exposed to pH 3.0-3.5 for 30 minutes to two hours. Surviving cells were counted by plating on blood agar but also examined using LIVE/DEAD® BacLightTM viability stain. Inhibition of the cell’s ability to induce an ATR was tested by exposing the cells to 0.5 M fluoride. Mature biofilm cells (3 days old) showed a different protein expression pattern compared to corresponding planktonic cells with the most prominent difference being the decreased expression of enzymes involved in carbohydrate catabolism. Contact with a surface induced changes in protein expression as quickly as two hours after surface adhesion. In these cells however, the proteins involved in carbohydrate catabolism were enhanced. Surface contact and growth in a biofilm lead to increased acid tolerance. In newly formed biofilm cells of S. mutans H7, 5% of the cells survived an acid shock of pH 3.0 for two hours compared to only 0.0004% of the planktonic cells. The mature biofilm cells were even more acid tolerant with 41.5% survivors after two hours at pH 3.0. Mature biofilms induced only a negligible ATR that did not enhance their survival significantly. Still, 20% of the proteins analysed had an altered expression after exposure to pH 5.5. In newly formed biofilm cells the ATR lead to decreased membrane damage at low pH as visualised by LIVE/DEAD® BacLightTM staining. Different strains showed different abilities to induce an ATR and it was also shown that this ATR could be inhibited by exposure to 0.5 M fluo-ride during the pH 5.5-adaptation period. The results of the thesis show that biofilm cells of S. mutans differ from their planktonic counterparts and the strains of S. mutans tested behave somewhat differently. Swedish abstract: Mikrobiologisk forskning har till stor del bedrivits genom att man odlat bakterier i näringslösningar och på agarplattor och sedan studera bakterierna på olika sätt. Detta sätt att odla bakterier överensstämmer inte särskilt bra med hur bakterier växer i verkligenheten. I naturen växer bakterier fastsittande på olika typer av ytor (tandemalj, epitel, stenar, plastytor etc.) i s.k. biofilmer. Det har visat sig att bakterier i biofilmer inte alls beter sig som bakterier som växer fritt i en näringslösning (planktonisk form). Biofilmbak-terier skiljer sig från planktoniska bakterier genom att de är mer resistenta mot antibiotika och biocider. De är mer stresståliga, de växer långsammare och de har ett annorlunda gen- och proteinuttryck. Sammantaget har biofilmbakterier andra egenskaper. På grund av detta, har fokus inom mikrobiologisk forskning det senaste decenniet riktats mot att studera biofilmer för att komma närmare bakteriernas verklighet. I munnen växer bakterierna i biofilmer. Bakterierna i munnen är nödvändiga för att god munhälsa men de är också orsak till två av världens vanligaste sjukdomar; karies och parodontit. Streptococcus mutans tillhör den normala munfloran men medverkar också till att karies utvecklas. Karies uppstår när det blir en obalans i det orala ekosystemet. När man äter sjunker pH i munnen pga att bakterierna i plack producerar syror när de bryter ner kolhydrater. Om man äter ofta kommer pH i plack att vara surt under längre perioder, vilket leder till en selektion av bakterier som är stresståliga och kan anpassa sig till den sura miljön. Detta resulterar i att en syratolerant mikroflora dominerar i placket. Dessa bakterier har i sin tur kapacitet att göra det ännu surare i placket under långa perioder. Detta kan leda till demineralisering av emaljen och upp-komst av karies. S. mutans anpassar sig till en sur miljö genom att inducera en syratoleransrespons (ATR) vid subletala pH-värden. Syftet med denna avhandling var att studera hur S. mutans beter sig när den växer i biofilmer och ta reda på mer om syratoleransresponsen i S. mutans biofilmer. Resultaten från avhandlingen visar att S. mutans bakterier som vuxit i en biofilm i tre dagar har ett annorlunda proteinuttryck än planktoniska bakterier och att förändringar i proteinuttryck sker redan efter två timmar av ytkontakt. Proteinuttrycket ändras efter hand som biofilmen växer och blir äldre. Biofilmbakterierna visade sig också vara många gånger mer syratoleranta än de planktoniska bakterierna och ytkontakt i sig var viktigt för den ökade syratole-ransen. Äldre biofilmer (3 dagar) var mer syratoleranta än unga biofilmer (3 timmar) vilket kan bero på att de utsatts för pH chocker under växt i biofilmen och därmed inducerat en ATR. Unga biofilmer inducerade en ATR när de utsattes för pH 5.5 i tre timmar och denna ATR kunde inhiberas genom tillsats av fluor. Det visade sig också att olika stammar av S. mutans var olika syra-toleranta och hade olika förmåga att inducera en ATR. Detta kan i praktiken innebära att människor med bakterier som är mer benägna att inducera en ATR löper större risk att utveckla karies. Genom att förhindra induceringen av ATR skulle man alltså kunna förhindra utvecklingen av karies.

Acid tolerant streptococci in dentine caries using pH-selective agars

English abstract: Objectives: To investigate the acid tolerant microflora in dentine caries, with special respect to oral streptococci cultivable on pH-selective agar media, as acid stress might be a major selective force in dentine caries. Methods: Five patients with primary dentine caries lesions (vital teeth, no symptoms) participated in the study. Caries lesions were excavated aseptically under rubber-dam using rose-burs. Dentine sampling was performed using sterile rose-burs at 3 levels: superficially, in the centre and in the cavity floor. Samples were incubated on neutral (blood agar) and pH-selective agars (Todd-Hewitt agar with citrate-phosphate buffer; pH 4.5, 5.0 and 5.5). Numbers of colony-forming units (CFUs) were determined, characterized morphologically, isolated to blood agar and then identified (Gram-reaction, cell- and colony morphology). Each colony type was frozen in skim milk (158 isolates). Isolates described as streptococci were revived, re-incubated (65 isolates) and further characterized by enzymatic- and sugar fermentation tests. Results: The same bacterial counts were recovered from superficial, center and cavity floor, respectively. Each dentine sample exhibited a unique microflora. There was no significant difference in proportions of aciduric microorganisms in different levels in the lesion. Approximately 38% (superficial), 5% (centre) and 38% (cavity floor) of the total cultivable microbiota was able to grow at pH 5.5, a pH value critical for demineralisation of dentine. The acid-tolerant streptococcal isolates included mutans streptococci, S. anginosus, S. constellatus and a group of unidentified streptococci. Lactate consuming taxa were found in one case, but only at pH 5.5. Conclusion: There was no significant difference in numbers of microorganisms on different levels in the lesion. Composition of the dentine caries microflora differs from lesion to lesion. The dentine caries microflora consortia differ from lesion to lesion. A common property of these bacteria was acid tolerance. Approved by the ethical committee Lund University. Funded by Faculty grants.

English abstract: Lactoperoxidase (LPO) is an enzyme, which is used as an antimicrobial agent in a number of applications, e.g., food technology. In the majority of applications LPO is added to a homogeneous product phase or immobilised on product surface. In the latter case, however, the measurements of LPO activity are seldom reported. In this paper we have assessed LPO enzymatic activity on bare and protein modified gold surfaces by means of electrochemistry. It was found that LPO rapidly adsorbs to bare gold surfaces resulting in an amount of LPO adsorbed of 2.9 mg/m2. A lower amount of adsorbed LPO is obtained if the gold surface is exposed to bovine serum albumin, bovine or human mucin prior to LPO adsorption. The enzymatic activity of the adsorbed enzyme is in general preserved at the experimental conditions and varies only moderately when comparing bare gold and gold surface pretreated with the selected proteins. The measurement of LPO specific activity, however, indicate that it is about 1.5 times higher if LPO is adsorbed on gold surfaces containing a small amount of preadsorbed mucin in comparison to the LPO directly adsorbed on bare gold.

English abstract: AIMS: To acquire a deeper understanding of adolescents' experiences of living with temporomandibular disorder (TMD) pain. METHODS: Twenty-one adolescents with TMD pain, aged 15 to 19, were strategically selected from a group of patients referred to an orofacial pain clinic. The patients were examined and received diagnoses per the Research Diagnostic Criteria for TMD. One-on-one interviews that followed a semistructured protocol focused on the patient's experiences of living with TMD pain. The interviews were recorded and transcribed verbatim, followed by content analysis to obtain a deeper understanding of adolescents' experiences living with TMD pain. RESULTS: Content analysis led to the overall theme "Adolescents with TMD live with recurrent pain; physical problems and daily demands form a vicious circle that causes adolescents to oscillate between hope and despondency." The latent content forming the theme is grounded in three categories that evolved from 13 subcategories. For instance, five subcategories-headache; headache on awakening; jaw and tooth pain; constant thoughts of pain; and popping, cracking, clicking, and locking-formed the category that was labeled TMD pain is recurrent. The latent interpretation, ie, the meaning, of this category was that adolescents with TMD pain constantly thought about the pain, even when it was absent. CONCLUSION: TMD pain is a substantial problem for affected adolescents and has consequences for all aspects of their lives. In this study, the adolescents were able to talk openly and introduce issues outside of the interview protocol. Qualitative analysis deepens our understanding of the adolescent patient with TMD pain. -
